LMI / Fox side bender assembly instructions

I purchased a side bender kit from LMI many moons ago and am just putting it together now. But I have lost the assembly instructions. This is one that they sold with the light bulbs. LMI Couldn't help me and I'm still looking. Can anybody help?

Daniel, I made one of the side benders from an LMI kit, about 10 years ago.  I couldn't find the plans, but did find an old LMI catalog that has 7 pages of instructions and tips for making and using the bender.  The article was written by Charles Fox, but a lot of the tips and suggestions were from Paul Jacobson.  Let me know if this would be useful to you.
