Guild 12 acoustic truss issue .

Hi everyone , a customer has a 12 string Guild , one of the truss rods is spinning . I will have to remove fretboard , does anyone have experience with the anchor points of the rods ? Thanks .

You can see in the first pic how the truss blocks of mahogany have let go and slipped into the channels . I cleaned things up and re-glued them , but even so they will only get the neck just straight , no back bow . Even without the fretboard . any ideas ? maybe a bad design or bad install ?
