Music man Stingray bass with truss rod pushing out fingerboard

Customer had tightened the dickens out of his truss rod , and now it has erupted between the nut and the first fret...I'll be doing some exploratory surgery in the next couple days...Wondering how it was supposed to be anchored....I loosened it all the way , and it would not clamp down...

Is there any chance you could post some more pictures of the situation? Taken from different angles (front/sides)

I did the surgery to expose the problem....It was only erupting from the nut slot to the first fret...When I got in , and worked the truss rod nut , it bottomed out on the spacer , and then the rod itself started to turn...I tried to push the spacer back down with clamp pressure , but nothing moved , even with no tension on it...The maple filler strip was also cracked...There was no filler strip over the spacer part...

There could be bits of junk under the rod or spacer keeping it from going back where its sposed'ta. I think I would concern myself with finding out why the rod itself is turning freely first, though.

I decided to yank the whole fingerboard and fix it , or replace the rod..
