I have a Martin D-15 that I play a lot. She's about 8-9 years old. I have a really bad habit of strumming too far towards the neck and have worn out the soundboard a bit above the pickguard (toward the neck). Nowhere near wearing all the way through, but eventually it will look like Willy Nelson's guitar. There is also wear around the sound hole. Should I get a custom pick guard installed? How much would that cost? Not something that I am interested in doing myself.

Sorry, I re-write my answer because I clicked on "delete". (I'm growing old!). So, here we go:
Any experienced repair man can do a custom pick-guard with the apposite adhesive sheet, in any shape. Just watch that the edges will be bevelled. I did many and rarely I charged more than 30€. It should depend on the complexity and dimension of the shape and quality of the material.
