I just dug through the archives.

Its a Hofner 4575

Factory specs in the Höfner book by Michael Naglav: Back and sides flamed maple, double cutaway. 1 1/4" high sides, spruce top, celluloid binding around top back and f-holes, fingerboard inlays celluloid, headstock inlay MOP. Electronics have frequency lifting.

It's a mid 60's ('65-'66) Hofner Verithin.  They're still well regarded in the jazz world. Nice find :)
